A NOTE ON FORMS

As a volunteer-staffed camp, our volunteers  sort through and log all the forms.  This takes a tremendous amount of time!  You can help us by:
  • Turning your forms in by the deadline (May 1st)
  • Turning in ALL your camper's forms
  • Fully completing the Health Form which has two parts - (1) Participant Health Form (Doctor's Physical) AND (2) Participant Health Form (Health History).
  • Participant Health Form - Dr's Physical is to be completed by AND signed by a physician/nurse practitioner within 12 months of camp.  This page then needs to be uploaded to the camper's ACTIVE account.
    1. For certain health plans, physicians will not fill out the Participant Physical page - instead upload the health examination record supplied by the physician to our camp's health form.  Be sure to include immunization dates.
  • Participant Health History is to be completed and signed by parents (pages 1-4) and then uploaded to the camper's ACTIVE account.  If you miss a part, our volunteers must catch the mistake, contact you, you must re-submit the form, and then our volunteers have to re-review the form.  Please save us all this time by noting the following common mistakes:
    1. Immunizations - YES, we DO need this ("all current" is not good enough!); but you can include a copy of their immunization record instead of filling out this section.
    2. Medications - if it is NOT on the health form we can not administer the medication to your child (even if you send the medication in the original container with the Rx label!).  If you do not mark an over-the-counter (OTC) medication as OK to give your camper, then we can not give it to them even if they need it.    Only medication in the original container and prescription (if applicable) label attached with a doctor's note can be administered at camp.  We have a well-stocked Health Center with qualified health supervisor's on site.  Over-the-Counter (OTC) medicines will be used to treat routine medical needs per Girl Scout's Treatment Protocols.  (NOTE: Acetaminophen is used in place of aspirin.)  There is a place on the Participant Health History for you to list any OTC medications you do NOT want given to your camper.
    3. Allergies/Food Restrictions - If your camper has a food, pollen insect, animal medication or other allergy, please not these on their health history form.  Please give a description of their reaction and the prescribed treatment.  We are able to accommodate most dietary needs only IF: they are noted on both the registration and health forms AND received by May 1st.  NOTE: Due to ordering constraints, a camper cannot change their dietary requirements while at camp based on their preference for that meal.
    4. An Email is Not Enough - if it is health and safety related, it needs to be on the Health Form.  An email to camp is not enough to ensure that all our staff have this vital information.  All staff review the health forms for campers in their care.  It needs to be on THIS form!
