Time to do Some Paperwork

ALL FORMS ARE DUE MAY 18TH

Girl Scouts of Northern California requires that every person attending camp MUST have a current health form, with physician signature, on file with our camp. That means that every year a NEW form must be submitted.

There are two ways to submit your forms:

Upload them to our Active system -OR-
Mail hard copies to Camp Butano Creek, 1771 Dawn Street, Livermore, CA 94550.

A note on the Health Form - it is NOT completed unless all parts are filled out. If you miss a part, one of our nurses will have to contact you, you will have to finish the form, then re-submit it, nurses have to then re-check it and log it if it is complete. Please save us all this extra effort by completely filling out the form (YES! we need a copy of your immunizations, YES! we need a doctor's signature, YES! we need all your medications listed, YES! we need you to sign the "permission to treat" part, etc.)
